
HMH Offers Voluntary Long-Term Care + Life Insurance
Eligible employees working 21+ hours per week up to age 64, have a one-time opportunity to apply with Guaranteed Issue, meaning no health questions. Newly eligible employees will have 30 days from their eligible start date to enroll.
Employees ages 65-75, those enrolling outside of the 30 day window and/or above the Guaranteed Issue maximum coverage will be required to answer health questions before coverage is issued.

Advantages of the Special Enrollment
Guaranteed Issue
Eligible employees ages 18-64 working 21 or more hours per week may enroll for up to $125,000 in coverage with no health questions. Eligible spouses aged 18-64 will answer some health questions. Employees ages 65-75 may enroll with health quesitons.
Accelerated Life Insurance for Long-Term Care
Accelerated Life Insurance can offset the costs associated with long-term care services not covered by health insurance, disability, or Medicare. Comprehensive coverage means you can receive the benefit whether you are receiving care in your home, an assisted living or skilled nursing facility, or require adult day care.
Life Insurance Benefits
A valuable life insurance benefit for your loved ones. This coverage is portable without penalty even if you change jobs or retire. This could be an important supplement to current life insurance that may terminate in the future.
